To provide an autonomous nurse led inflammatory bowel disease service in Lewisham and Greenwich Healthcare NHS Trust (Cross Site Working) ensuring delivery and expansion of an expert and advanced quality nurse-led service through effective interdisciplinary care, utilising clinical and managerial expertise. Provide education and support with the aim of improving access to treatment services.
The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards within the service. Including development of policies, protocols and guidelines work collaboratively with all staff within the multidisciplinary team, providing support, advice and formal education. To ensure the safety and wellbeing of services users, and always working in a manner that promotes dignity and respect thorough patient centered care principles. To act in an advisory capacity and as point of contract for patients with Inflammatory Bowel Disease conditions.
To contribute to the service development of inflammatory Bowel disease, within gastroenterology including clinical governance and audit.
Though based at University Hospital Lewisham, this is a cross-site role and the post holder will be required to work cross-site to deliver the expected outcomes for our patients
Provide nurse led clinics that will deliver direct nursing care, monitoring the efficacy and compliance of patients undergoing therapy.
Assess, monitor and evaluate disease activity and drug therapy of patients by initiating and interpreting clinical data
Act as an advocate to empower patients, their families and other carers to participate in decisions regarding their care.
Ensure optimum liaison and co-ordination of care for patients between primary and secondary care.
Identify the physical, social and psychological needs of patients experiencing gastroenterological conditions and monitor outcomes.
Document and maintain accurate records in line with professional standards of record keeping.
Contribute to the development and updating of nursing policy, guidelines/protocols and integrated care pathways with the gastroenterology service and ensure a high standard of care through ongoing review.
Undertake nurse led clinics for patients accepting referrals from Consultant colleagues and admitting and discharging patients from own caseload as appropriate.
Provide expert clinical information and advice to patients, carers and GPs through telephone advice lines / email
Provide effective teaching to individual patients and their families about their condition, treatment options and possible side effects.
Prioritise own workload and manage time effectively. Identify priorities and integrate with existing support networks.
