A high profile, accessible and responsive leader with managerial responsibility for the delivery of the Critical Care Outreach Service within the Trust. Service provision will facilitate improvements in the quality and provision of services through collaboration between Multidisciplinary Teams and other service agencies involved in caring for acutely ill patients. The post holder will lead service developments and act as a resource for other healthcare professionals, improving the quality of care provision whilst carrying a specialist caseload of clients and providing advice to patients, carers and the wider multidisciplinary team.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
- Accountable for own actions in accordance with Code of Professional Conduct. Will contribute towards corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
- To operationally lead the Critical Care Outreach Team and to provide advanced clinical expertise, knowledge and skills utilising clinical judgement and decision making in the care and management of acutely unwell patients.
- Highly developed communication skills enabling maintenance of high quality service and ensuring timely management and resolution of unexpected events and situations and conflicting priorities
- Coordinate the care of critically ill patients through the critical care pathway to ensure the effective management of their patient group.
- Responsible for identifying and assessing complex clinical and non-clinical issues, implementing resolutions and monitoring/auditing outcomes..
- Undertake advanced health assessment using history taking skills and advanced physical assessment knowledge to ensure optimum patient management including; patient monitoring; equipment management and defined expanded clinical roles.
