We are looking for a dynamic Leg Ulcer and Lower Limb Specialist to join our Lower limb ,lymphoedema and leg ulcer service at Lewisham and Greenwich NHS Trust. The registered OT, Physiotherapist or nurse specialist will work across the borough alongside the leg ulcer and lymphoedema nurse specialists ,District Nursing and wider multi disciplinary teams to deliver high quality, safe and effective care, ensuring excellent patient experience.
The post holder will be based at the Waldron Health Centre, New Cross and will be expected to travel within the borough of Lewisham as necessary.
A full UK driving licence and daily access to a car is essential.
The post holder will provide a safe, effective, high quality specialist service to patients their families and carers and promote self-management of the patient’s condition.
You will take designated responsibilities for a defined caseload and assist teams with assessing patients in their own homes, carrying out holistic assessment, planning, implementation and evaluation of nursing care working as part of the Community Nursing and GPs and Practice Nurses, liaising and working with statutory and voluntary agencies as appropriate.
