We are looking for an experienced specialist health care professional who is self-motivated, dynamic and forward thinking to support patients with lipid disorders and endocrine conditions within Cwm Taf Morgannwg University Health Board. The Lipid/Endocrine Specialist Nurse will act as an expert resource providing specialist advice and support innovation, improved access, and high-quality patient care.
The post holder will work collaboratively with the Diabetes Team, ensuring patient care is delivered to the highest standard, utilising research and evidence based practice in line with local and national policies, professional standards and guidance.
- Offer specialist advice on lipid and endocrine care to colleagues and develop professional networks locally and nationally.
- Provide empathetic support to patients and carers, often dealing with anxiety, chronic illness, or complex emotional needs.
- Share clinical findings and treatment plans clearly through verbal updates, reports and letters.
- Represent the service in team and professional meetings, promoting coordinated, multidisciplinary care.
- Support and evaluate service development projects and health improvement programmes
This post is fixed term/secondment for 18 months due to - meet the needs of the service.
