The role of the acute medical Geriatrician has been developed over the last 5 years to provide specialist Geriatric medical input to frail older people within the acute medical admission pathway. Our experience shows that patients who are on the Geriatric Assessment Unit, working with a multidisciplinary team results in more frail older people being discharged earlier in the pathway.
We in-reach into the Emergency Department and work alongside the emergency physicians in identifying frailty at the front door. We have also been part of the Acute Frailty Network and labelled a beacon site by them. More consultant geriatricians are being appointed to enable us to deliver this service more consistently across the week. We expect these roles to develop and change over the coming months and years as the acute medical pathways are reviewed so they are better able to meet the increasing demands being put on them.

Over the past five years, Nottingham University Hospitals NHS Trust (NUH) has led the way in developing the role of the Acute Medical Geriatrician—placing specialist expertise at the centre of the acute medical admission pathway. This is a role with real purpose and impact.
Through our Geriatric Assessment Unit (GAU), we’ve demonstrated how early, targeted geriatric input improves outcomes—enabling frail older adults to be assessed, treated, and discharged earlier, avoiding unnecessary hospital stays and ensuring care is tailored to their needs.
Our consultants are embedded in the Emergency Department, working closely with frontline teams to identify and manage frailty from the point of arrival. This proactive approach has earned NUH recognition as a “beacon site” by the Acute Frailty Network.
We are now expanding our consultant team, with a clear commitment to delivering this high-quality service consistently across the week. As demand grows, these roles will continue to evolve—offering opportunities for leadership, innovation, and shaping the future of frailty care.
