Job Summary
An exciting opportunity has arisen for a Community Support Worker to join our Neighbourhood Mental Health Team (NMHT). The NMHTs are at the forefront of providing accessible care, integrating both primary and secondary mental health services, in line with NHS long-term objectives. There are 5 Neighbourhood Teams in Herefordshire. The post advertised is to work within the South & West NMHT. Each NMHT is comprised of Mental Health Practitioners, Support Workers, Administrators, Clinical Psychologists and a Consultant Psychiatrist. Additionally, the teams are supported by Mind Link Workers and Vocational Employment Specialists from Landau, with the aim of providing a holistic, recovery-focused service. We work closely with our Primary Care Networks, demonstrating good relationships and communication with our respective GP partners. The teams are supportive, with regular MDT meetings, Individual/group supervisions and team away-days. For more information please contact the Recruiting Manager.
This is a full time role.
Please note the Trust will not routinely provide sponsorship to work for this role. In addition to meet Home Office salary threshold you must have the significant experience to be offered top of the pay band AND work a minimum of 35.5 hours per week.
Main duties of the job
To provide a range of patients with mental health support in the community, implementing plans of care, utilising personal autonomy and utilising the support of the Multi-disciplinary Team (MDT) when needed, to support individuals with their ongoing recovery.
To work with a range of agencies within the NMHT and externally, to sign-post patients to appropriate sources of support or provide short, moderate or longer-term interventions to support their recovery, based on their clinical need.
The post will require working with the team to support the running of the service, managing a caseload, attending meetings, and taking on additional responsibilities as and when required. The ability to be flexible and organised is essential. Experience of working with individuals on graded exposure work is preferable.
To be able to travel throughout the county as required to provide patient care and multi-agency working. To also provide support to those in a caring role.
