Job Overview

We are seeking an experienced, motivated and highly skilled Allied Health Professional to join the North West London Neuro Navigation Service as a Band 7 Neuro Navigator, as part of an exciting expansion of this developing service.

This pivotal clinical role supports adults with complex neurological conditions as they move across the whole rehabilitation pathway, from acute admission through specialist rehabilitation and into long-term community or placement settings. The post holder will provide expert clinical assessment, proactive case management and specialist neurorehabilitation advice, working closely with acute teams, rehabilitation providers, community services, social care and the voluntary sector to ensure patients access the right care at the right time and to enhance system flow.

You will manage a complex caseload, work autonomously, and act as a key point of contact for patients, families and professionals, requiring excellent clinical reasoning, communication and partnership-working skills. The office base for the role is Uxbridge; however, the post will predominantly involve regular travel to hospital and community settings, with community bases utilised across North West London. Main duties of the job

• Act as a senior clinical link across acute, rehabilitation and community services for adults with complex neurological conditions.

Provide expert clinical assessment and neurorehabilitation advice, supporting identification of the most appropriate rehabilitation pathway, including level 1, 2a, 2b, community and out-of-area provision.

Facilitate and monitor referrals and rehabilitation admissions, supporting patient flow and coordinated discharge planning.

Work in partnership with acute teams, rehabilitation providers, continuing healthcare, social care, commissioners and the voluntary sector to promote integrated, person-centred care.

Provide a proactive case management approach for patients with complex rehabilitation and discharge needs, acting as a central point of contact for patients, families and MDTs.

Provide specialist advice, education and training to health and social care professionals to support best practice and pathway understanding across the system.

The Neuro Navigator post holder will have a clinical case management role. They will support clients with neurological disabilities as they move from acute services through the whole pathway until they are settled in a permanent/long-term setting.

The Neuro Navigator will:

• Act as a link person for acute and community colleagues, rehabilitation units and GPs and the continuing healthcare team understanding pathways accessed by all clients with a neurological diagnosis (both single neurological events such as stroke and clients with neurological conditions). They will have a role in liaising and networking with neurological services.
• Provide an expert assessment of neuro rehabilitation need, providing a solid understanding of the clinical impact of stroke and acquired neurological conditions.
• Work with partners where an assessment is required to ensure robust completion of appropriate assessments, clarity of changing need and appropriate management of expectation with the family regarding expected outcomes.
• Provide an expert neuro rehabilitation opinion on the appropriateness of rehabilitation referral supporting identification of an appropriate pathway, working in partnership with health and social care professionals to promote an integrated approach to management of progressive and non-progressive neurological conditions.

Have an overview of challenges in the system impacting flow and supporting clinical decision making.

The post holder is a lone practitioner within this specialist field in Northwest London. They work autonomously within the standards of professional practice laid down by the relevant professional body and Healthcare Professional Council (or equivalent).

Qualifications And Training

Essential criteria

• Graduate Allied Health Professional Degree
• Current & Registered with Health Professions Council or other relevant governing body.
• Evidence of recent CPD and education within the field of neurorehabilitation
• Member of a relevant professional body
  
  

Desirable criteria

• Educated to master's level in Neuro Rehab related field or equivalent combination of post graduate education, courses, and experience.
  
  

Experience

Essential criteria

• Extensive post-registration clinical experience at a senior level within neuro rehabilitation
• Experience of working in a variety of acute and community settings

Extensive experience of undertaking complex assessments and providing care to people with Neurological conditions

Experience of effective inter-disciplinary and multi-agency work across organisations.

Teaching / training of undergraduate / postgraduate/staff and other professionals

Experience in completion of audit or service development projects

Experience of planning and delivering mentoring, supervision, and appraisal to clinical and non-clinical staff

Desirable criteria

• Establishing or developing innovative clinical standards/pathways
• Experience of undertaking research
• Evidence of working with local authority and homeless and inclusion teams.
• Awareness of funding streams such as Continuing health care.

Essential criteria

• In-depth knowledge of Neuro Rehabilitation and neurological presentations
• Expert knowledge of current guidelines and national standards relating to neurorehabilitation

• Understanding of current NHS and local / national policies and clinical governance

Confidence and assertiveness in a challenging environment

• Experience of collaboration, advanced communication skills and close working across different teams and sectors
