ute Hospitals which includes the Emergency Departments, assessment units and all wards to proactively identify patients who will need Onward care on discharge/transfer and to support the multi-disciplinary team with facilitating complex discharged aiming for home first.
Take the lead in discharge arrangements for individual patients where appropriate liaising with all members of the multi- disciplinary teams in the acute setting and providers in the community.
Act as a resource for patients, relatives and carers requiring information on discharge processes, services available and all other issues relating to discharge and admission avoidance in accordance with current legislation.
Adopt a Home first approach, enabling patients to return safely home earlier from hospital.
Assess patients for suitable and safe transfer to community hospitals, community nursing teams and community provider services, both in and out of county.
