Outreach Caseworker
Location: Chelmsford, Essex
Salary: £30,000 per annum
Hours: 35 per week
Contract: Fixed term contract (6 months)
Additional Requirement: Applicants must have a full UK licence (held for 2 years) and access to a vehicle.

As an Outreach Caseworker, you’ll work directly with individuals who frequently access urgent care services. You’ll help them identify unmet needs, build coping strategies, and connect with the right support. This role is vital in creating a more compassionate, efficient, and person-centred care environment.
