We are looking to appoint an enthusiastic and motivated person to join the Women's Services Team at Ysbyty Gwynedd as a part-time Patient Pathway Tracker initially for a 6 month period.
The Patient Pathway tracker role includes management of the patient journey through the care pathway in accordance with Welsh Government Referral to Treatment (RTT) guidelines and requirements.
The successful candidate will have good organisational skills and a good knowledge of the RTT guidelines. They will work closely with the Operational Management Team and liaise with the administrative and clinical staff to progress patient pathways and ensure accuracy of surgical waiting lists.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
To ensure progression of the patient journey through the Care Pathway in accordance with Welsh Government Referral to Treatment (RTT) guidelines and requirements. The post holder will work closely with the Women's Services Operational Management Team and will liaise with Administrative and Clerical Staff to progress pathway and ensure accuracy of waiting lists throughout all Stages of the Patient Pathway producing reports for Management meetings to demonstrate this progression.
Key Tasks And Responsibilities
To source, collect and validate accurate RTT data from differing PAS systems, in accordance with agreed practices and standards;
To ensure RTT (Referral to Treatment) data reported to the Women's Site Management Team is accurate and delivered in a time efficient manner for submission;
Investigate and interrogate the Health Board’s PAS systems efficiently and accurately to provide RTT status, eliminate errors as identified and ensure any corrections that are identified are forwarded to the relevant department for action.
To actively support the Management Team in identifying, reporting and resolving department issues in order to achieve a high standard of data quality and completeness of RTT data;
To attend relevant meetings across the Health Board sites and contribute as a RTT expert as required This will entail travelling across North Wales;
To work closely with Cancer Services Department to assist in the delivery of the 31/62 day pathway

Essential criteria
- Diploma Level Qualification in Administrative Related Field (or equivalent level of experience)
- English Language and Maths GCSE Standard
Desirable criteria
- ECDL
- Stage 2 Word Processing and or Typewriting
Experience
Essential criteria
- Good knowledge of current Referral To Treatment/Cancer Targets
- Previous NHS experience
- Experience of data handling including sourcing, collecting and interrogating of information
- Experience of interaction with all levels of staff
Desirable criteria
- Advanced knowledge of Excel
