Based on its award-winning Rehabilitation Model, Cardiff and Vale UHB Therapy Services have developed a Live Well Service to work with people with long term health conditions. A key element of this service is co-production and harnessing community power in supporting individuals to live well. We already have a well-established and successful co-production forum; it is an exciting time as we work together with our community to extend our service and in bed co-production and lived experience expertise within the services we deliver. We are looking for a highly motivated individual with knowledge and skills in co-production to help us, and the other co-production lead continue to take our vision further.
The co-production lead will be responsible for bringing our vision to its full potential. They will co-ordinate, lead and continue develop the existing co-production forum establishing effective relationships and networks extending our reach into the population of Cardiff and Vale. Utilising co-production methodology, they will support the development of new groups and peer support activities in the community, following up actions, ideas and opportunities created in the co-production forum. Through community presence this role will create, manage and support new connections and wider engagement of our community partners with a focus on personal wellbeing and peer support. The lead will support peer practitioners within the Live Well Service and work alongside, the Cardiff and Vale UHB Lived Experience Team, Recovery College and others to develop a bespoke training programme for wider therapy service peer supporters/practitioners.
You will support colleagues in developing the way we deliver our services. developing their recovery focussed practice and embedding a co-production ethos at every level in our services. The lead will apply their specialist knowledge of co-production to guide the development of this ethos, its application within our services and its governance within a clinical framework.
You will evaluate impact and challenges. Reporting and be supported within the framework of the Rehabilitation Model Steering Group.
