Are you a Band 5 Physiotherapist, newly qualified or looking to progress and build further knowledge and skills in a community setting?
We are looking to recruit a Full Time rotational Junior Physiotherapist to join our Community Therapy Team in Guildford and Waverley. The rotations currently are MSK out-patients, In-patients (sub-acute ortho-geriatric, frailty), and Community Therapy Team, who see both urgent and general rehabilitation, including patients with neurological impairments.
You will be part of a friendly and experienced team of Physiotherapists, Occupational Therapists, PTAs and Community Rehabilitation Assistants. You must be willing to learn to use IT applications to carry out this job effectively. You will need to be able to travel independently between sites and to patients’ homes.
The In-patient, MSK and Community Therapy Teams aim to provide high quality, responsive physiotherapy services, for patients with a wide range of complex physical impairments including long-term conditions and frailty. The community patients may be at risk of admission to hospital or have recently been discharged from hospital. You will need to be able to work autonomously as well as part of a multidisciplinary team and be able to promote innovation and improvement within the community therapy team.
The Band 5 physiotherapist will provide intervention for referred patients as part of a multi-agency approach to meet the health and social care needs. He/she will be responsible for a defined group of patients and will be responsible for providing assessment, treatment and advice to these patients. You will work across the Community service rotation programme.
You will assess and treat people in bedded units, in an out-patient setting and in their own homes including using telephone and video calls, and will manage your own caseload of patients. We work closely alongside other community services from health, social and voluntary sectors to provide the best service to enable patients to have the best possible quality of life.
