We have an exciting opportunity for an experienced and innovative pharmacist to lead our progressive Pharmacy Formulary and Interface team.
You will manage the delivery of formulary services at GSTT and the South East London Joint Formulary.
You will work across systems to deliver our local, regional and national priorities.
You will act as the main secondary care lead for formulary and medicines pathway in the South East London Integrated Medicines Optimisation Committee (IMOC).
The Main Duties Of This Role Are
- Management of the South East London Joint Medicines Formulary, delivery of formulary services at GSTFT to support safe, clinically effective and efficient use of medicines in all situations
- Communication and implementation of formulary management matters both within the Directorate, throughout GSTFT, and to King’s College NHS Foundation Trust (KCH), Lewisham & Greenwich NHS Trust (LGT), South East London ICB and other healthcare providers (e.g. South London and the Maudsley NHS Trust).
- Assistance and supervision of all formulary and regional medicines pathway and interface matters for KCH and LGT
- Development and maintenance of effective working relationships with colleagues in primary and secondary care across the locality.
Guy’s and St Thomas’ Pharmacy Formulary and Interface team is part of a large, vibrant service focussing on delivery of advice related to Medicines Effectiveness and Safety.
We aim to drive best practice through robust governance frameworks, collaboration and quality improvement. We promote the safe, effective and economic use of medicines in patients, through active provision and support with clinical governance processes to ensure good quality care.
Guy’s and St Thomas’ Pharmacy Department is one of the largest in the UK and has a strong focus on individual patient care. Our pharmacists work across multi-disciplinary teams both in the hospital and community setting, to deliver advice, guidance, and support within a culture of medicine safety. The focus is on patient-centred care and this is achieved through providing optimised pharmaceutical care for patients as part of a ward-based and clinical pharmacy service and extending pharmaceutical services beyond traditional boundaries. This includes attendance on consultant-led ward rounds and contributing to multi-disciplinary meetings.
