Join a pioneering team that brings hospital-quality care directly to patients' homes, transforming healthcare delivery across the community.
Brent Urgent Community Response Service is a national leader in intermediate care that aims to bring the best of hospital care to patients' homes, therefore avoiding unnecessary hospital attendances and admissions.
Most of our referrals come from GPs, London Ambulance Service, and Accident and Emergency departments. Attending in pairs, we perform an initial assessment at home, conducting a holistic evaluation of the patient and in some cases starting a treatment plan within 2 hours of referral.
The team can utilise a number of pieces of diagnostic equipment in the home, plus we have access to hospital facilities for various scans when required. We also provide our services in A&E to avoid hospital admissions and facilitate safe discharges.
Brent Urgent Community Response looks to provide the very best care, aiming to deliver the same standard a patient would receive as an inpatient. This is facilitated by having real-time access to patient records in the community via laptops.
We are looking for practitioners who have a passion for exceptional care, who want to develop their skills and push themselves outside the normal boundaries of practice, and who have a real drive to ensure their patients receive the best treatment possible. Excellent communication skills are essential.
