Are you passionate about supporting individuals to regain their independence and improve their quality of life? West Berkshire Council is seeking a dedicated and motivated Reablement Practitioner to join our Adult Social Care Service, working within the Reablement Team.

The Role

Responsibilities

As a Reablement Practitioner, you will be responsible for:

  • Delivering high-quality reablement support to individuals, enabling them to achieve their personal goals and maximise their independence.
  • Working collaboratively with service users, their families, and other professionals to assess needs and develop appropriate reablement plans.
  • Providing practical assistance and guidance in daily living activities, such as personal care, meal preparation, and household tasks.
  • Monitoring progress and providing regular feedback on individuals' achievements and areas for further development.
  • Contributing to the evaluation of reablement outcomes and participating in multidisciplinary reviews as required.

The Ideal Candidate

The ideal candidate will possess the following skills and attributes:

  • A genuine passion for supporting individuals to achieve independence and improve their quality of life.
  • Willing and able to deliver person centred care which may include a range of personal care tasks in a manner that is respectful and dignified.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, families, and colleagues.
  • Strong assessment and observation skills, with the ability to identify individual needs and set achievable goals.
  • Ability to work both independently and as part of a multidisciplinary team, demonstrating flexibility and adaptability.
  • Willing to work with individuals to develop their independence.
  • Willing to undertake training to develop skills to deliver effective reablement support.
  • Ability to work the below shift pattern:
  • Week 1: Monday 7am-10.30am, 12pm-1.30pm & 6-10pm, Tuesday 7am-10.30am, 11am-1.30pm & 6-10pm, Wednesday off, Thursday off, Friday 7am-10.30am, 12pm-1.30pm & 6-10pm, Saturday 7am-10.30am, 12pm-1.30pm & 6-10pm, Sunday 7am-10.30am, 12pm-1.30pm & 6-10pm.
  • Week 2: Monday off, Tuesday off, Wednesday 7am-10.30am, 11am-1.30pm & 6-10pm, Thursday 7am-10.30am, 12pm-1.30pm & 6-10pm, Friday off, Saturday off, Sunday off.

We would consider applications from those who wish to apply for only the morning and lunchtime hours or only the evening hours.
