We are looking for a
Recovery Coordinator to join our team in
Reading. You will work across custodial and community settings, ensuring continuity of care and support for individuals engaged in the criminal justice system, particularly those under MAPPA (Multi-Agency Public Protection Arrangements) and IOM (Integrated Offender Management). Your role will focus on identifying, engaging and supporting service users with drug and/or alcohol needs, delivering targeted interventions and robust release planning to reduce substance misuse, re-offending and associated harm. You will work closely with prisons, probation services, and community-based providers to ensure seamless transitions and timely access to appropriate support on release.

Hours Full time, 37.5 hours per week
- Full Time Salary Range £27,861.26 - £32,002.35 dependent on experience (based on full time hours, pro rata for part time hours)
Contract Fixed Term Contract until the 31st of March 2026
- Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About the role
- Deliver targeted, trauma-informed interventions tailored to the needs of individuals under MAPPA/IOM frameworks and ensure pathways into appropriate treatment services post-release.
- Build strong, effective working relationships with prison healthcare, probation officers, MAPPA panels, and IOM teams to support integrated care planning and information sharing.
- Act as a key liaison between prison in-reach teams and community drug/alcohol services to support continuity of care, shared case management, and coordinated risk reduction planning.
- Attend and contribute to MAPPA Level 2 and 3 meetings and IOM case reviews, providing written and verbal reports, including risk updates and recovery progress.
- Carry out prison-based screenings, initial assessments, and risk assessments, developing recovery plans in partnership with service users prior to release.
- Provide harm reduction education, overdose prevention, and relapse management planning with service users in custody and following release into the community.
- Deliver and promote Naloxone provision pre- and post-release, and train service users and peers in its use where appropriate.
- Offer “through-the-gate” support including gate pickups where appropriate and facilitate access to community recovery services, housing, and health appointments upon release.
- Conduct home visits, hostel visits, and community outreach as needed to maintain engagement, particularly with high-risk individuals and those at risk of disengagement.
- Work collaboratively with police, probation, mental health, housing, and social care partners to address the complex needs of high-risk service users and promote recovery and rehabilitation.
