Job Overview

A great opportunity has arisen to join our Medical Same Day Emergency Care Team on a fixed term basis!

Medical SDEC is made up of a team of dynamic nursing staff, doctors, advanced practitioners, pharmacist's and specialist nurses. The unit is fast paced with an emphasis on high quality rapid assessment and accepts patients from a wide variety of specialties.

The unit accepts referrals form GP’S, the Emergency Department, and Southwest Ambulance service, as well as working closely with NHS at Home. Following the expansion of the unit, we now provide speciality services within Acute Medicine, Respiratory Medicine, Infectious Disease, Gastroenterology, Endocrinology, Stroke, and Neurology.

Same Day Emergency Care (SDEC) remains at the forefront of NHS England’s urgent, emergency care recovery plan. The unit has received significant investment allowing the service to transform and grow over the last 2.5 years. The purpose designed unit now assesses 35 -50 patients per day.

Under this care model, patients presenting at hospital with relevant conditions can be rapidly assessed, diagnosed and treated without being admitted to a ward, and if clinically safe to do so, will go home the same day their care is provided.

Our dynamic, innovated approach to growing the service, along with a excellent patient feedback led to SDEC winning the trust’s “we transform services” 2023 award, something we are extremely proud of!

Main duties of the job

• To work as a member of the SDEC Team and be responsible for the assessment, development, implementation and evaluation of programmes of care.

To work in collaboration with the nurse in charge and clinical matron to ensure the delivery of high caliber of evidence based patient care, and to encourage and develop a flexible work force through the efficient use of facilities, manpower and other resources.

Take personal responsibility for ensuring Trust and local policies and procedures are known and followed and be familiar with advisory guidelines, as laid down by the NMC.

Supervise junior staff and to teach qualified staff, student nurses and healthcare support workers.

Be able to rotate into the emergency zone as required.

Patient Care

Detailed job description and main responsibilities

• Take personal responsibility for the nursing care of patients in the clinical setting without direct supervision.
• Ensure that care needs are assessed, and programmes of care are developed, implemented and evaluated for a patient caseload. Ensure care is documented accurately at every stage of the process, giving a clear profile of care from admission to discharge.
• Plan and evaluate own working practices.
• Take charge of the unit on a day-to-day basis as required and for a minimum of one shift per week.
• Provide advice and support to junior team members to enable them to carry out delegated duties and understand responsibilities.
• Ensure maintenance of clinical competence/skills of self and recognise shortfalls/developmental needs, feeding this into the individual development performance review (IDPR) process.
• Ensure clinical competence/skills meet the requirements of Trust Policies and Procedures and are evidence based.
• Ensure all clinical care requirements of patients are communicated effectively at ward handover to allow seamless provision of care from shift to shift.

Assist in implementing recommendations of audits.

Education

• Contribute to the facilitation of a suitable educational environment in which to support the development and learning of ward team members and students on placement.
• Act as mentor/assessor to students and/or others on placement.
• On behalf of the Unit Manager, act as a Link Nurse to specialist groups, ensuring feedback to ward team enabling review of practice.
• Participate in teaching students and others as appropriate.
• Act as a Green/Orange Card trainer for clinical equipment.
• Be responsible for using equipment only after relevant training and ensuring that equipment is checked prior to use.

Education/Training/Qualifications

Essential criteria

• Registered Nurse with current live NMC registration
• Evidence of continuing professional development relevant to the role
  
  

Desirable criteria

• Teaching and assessing qualification (FLAP or equivalent) or evidence of experience within mentorship and or teaching
  
  

Work Experience

Essential criteria

• Completion of training for additional clinical skills required in the ward/department i.e. IV drug administration, cannulation & venipuncture, male catheterization, blood transfusions, ECG training,

Recent, post qualification experience in an acute specialty

Desirable criteria

• Experience within Acute medical Admissions, SDEC or the Emergency Department
  
  

Knowledge/Skills/Abilities

Essential criteria

• Advanced communication skills, verbal & written, to include report writing and record keeping
• Basic computer / IT skills and expectation to develop these in line with the needs of the service i.e. IHS, Acis, Email use
• Ability to work as part of a team
