Interested in an exciting clinical prescribing role working to delivery the new Integrated Neighbourhood model of care within Lewisham Borough as part of the Lewisham Integrated Medicines Optimisation Service (LIMOS)?
We are looking for three enthusiastic senior pharmacists with strong established clinical skills to join the Lewisham Integrated Neighbourhood Team Programme and improve outcomes for patients with multiple long-term conditions in Lewisham.
INTs are a model of care designed to meet the holistic needs of the local population, working with professionals and as a local community to ensure people get the right care at the right time, from the right people and to tackle health inequalities. These posts will be key in ensuring delivery of this new model. As the programme will focus on patients with three of more long term conditions around CVD, diabetes and AF in the first instance, experience of management of patients in these areas would be beneficial.
The Senior Clinical Pharmacist Prescribers will be responsible for providing clinical care to conduct patient facing multi-morbidity reviews and optimise medicines management for patients across one or two Neighbourhood footprints. A key part will be close working with and supervision of non-clinical members of the core INT team including caseworkers, Health Coaches and link workers. Close working and communication with these members of the core INT and wider teams across both health and social care and primary and secondary care settings to ensure effective patient management and delivery of the INT programme will be key.
