Description
We are seeking an experienced and motivated Senior Community Mental Health Nurse to join our Home Treatment Service in Taunton. This service provides intensive, short-term support for people experiencing a mental health crisis, enabling them to remain at home as an alternative to hospital admission or to facilitate early discharge from inpatient care.
You will work within a friendly, multi-disciplinary team committed to delivering safe, effective, and compassionate care in line with the Triangle of Care principles. The role involves working flexibly across a 24-hour, 7-day-a-week service, undertaking rapid assessments, care planning, crisis interventions, and liaising with a range of professionals and agencies.
