Intensive Rehabilitation Service (IRS) – a multidisciplinary community team that plays a vital role in reducing hospital admissions and supporting early discharges.
As a Senior Physiotherapist, You’ll
- Manage your own caseload of community-based patients
- Deliver high-impact, person-centred rehabilitation in patients’ homes
- Collaborate with inpatient and acute teams to ensure seamless transitions of care
- Provide supervision, training, and joint sessions with colleagues across the team

Working as part of the Intensive Rehabilitation Service (IRS) to provide high quality, timely rehabilitation for patients requiring step up from the integrated health team or step down from an acute or community bed.
Delivering high quality rehabilitation in a patient’s home rather than in a community bed setting for those patients with a rehabilitation need as part of a multi professional service/team.
Working with patient centred goal orientated rehabilitation to promote self-management and independence throughout the continuum of care.
Providing onward referrals to other community services including health and social care to ensure the safe management of the patient when completing their IRS episode of care.
The post holder will be a Registered Physiotherapist (HCPC) responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.
The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.

High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement of 15%of the basic salary (minimum payment of £4,714 and a maximum payment of £5,941 - per annum, pro rata for part time
