Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings.
Our service consists of Health and Social Care colleagues.
Consultants, GPs, Advanced Care Practitioners, Social Workers, Nursing, OT, Physiotherapists, Discharge Team and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, frail patients.
You will be based across our 4 community bases in addition to our new purpose-built Hub at Harrogate Hospital, working with, colleagues from social care and our Acute Therapy Team.
- To carry a variable clinical caseload of patients within the Acute Response and Rehabilitation in the Community and Hospital Service (ARCH) providing complex physiotherapy assessments addressing physiotherapy performance, skills deficits, self-maintenance and cognitive function.
