Senior Sister Older Persons Medicine

Are you a passionate and experienced nurse ready to make a real difference in the care of the Older Persons?

We’re looking for an enthusiastic, highly motivated and experienced Nurse with good communication skills to join our dedicated Older Persons Medicine Team at Frimley Health NHS Foundation Trust. This is a rewarding opportunity to be part of a supportive and dynamic multidisciplinary team, delivering high-quality specialist care to patients.

Main duties of the job

  • Continually assess, plan, implement and evaluate evidence-based programmes of care to ensure the provision of a high standard of care.
  • Personally deliver a high standard of care to a group of patients, acting as a team leader / named nurse and clinical role model at all time.
  • Ensure the provision of advice, support and health information to patients and their carers to promote a health-based service
  • Manages people and/or items such as equipment or specimens safely and to time consistent with legislation, policies and procedures.
  • Maintain accurate records of all clinical and legal documentation to include accident / incident reporting, according to the NMC, statutory and Trust polices.
  • Ensures all aspects of clinical care including interventions and investigations are performed, assessed, and reported upon in a timely fashion and in a manner consistent with legislation, policy and procedures.

Detailed Job Description And Main Responsibilities

  • Responsible for the assessment of patients care needs the development, implementation and evaluation of programmes of care without supervision.
  • To participate and assist in systematic monitoring and evaluation of nursing practice.
  • To participate and assist in setting standards of patient care based on sound evidence-based findings, implementing these into practice.
  • Ensure that the nursing practices of the area comply with the Trust’s Nursing policies and procedures and maintain high standards of nursing practice.
  • To always comply with the NMC Code of Conduct (2008).
  • Communicate effectively and liaise with all health care staff as appropriate.
  • To encourage and maintain good relationship with Medical Staff and other members of the Primary care & Social Services teams to provide the best care.
  • To take responsibility for a caseload of patients. Within this system of case management, co-ordinate and monitor the progress of patients along their pathway, reporting any variance from the pathway.
  • Take named responsibility for co-ordinating individual patient’s discharge from the unit.
  • To participate in Quality Assurance initiatives within the unit, implementing action because of these initiatives.
  • Consolidate and extend own clinical skills in all aspects of nursing, furthering own professional knowledge in a rapidly changing environment
  • To ensure that all issues relating to the implementation of clinical governance are addressed on the unit. This includes maintaining and reviewing the standards of care on a regular basis, implementing changes to improve the service where necessary following complaints, ensuring systems are in place to manage all risk and to regulate professional practice. All staff must be made aware of their own individual responsibilities as regards clinical governance.

  • Teaching certificate

  • Qualification in older people's nursing

Skills & Knowledge

  • Good communication skills
  • Awareness of current professional/NHS updates
  • Teaching and assessment experience

  • Excellent leadership skills

Experience

Essential criteria

  • Recent experience in Band 6 in Acute setting
  • Evidence of up to date standards of clinical practice
  • Experience in completing audit
  • MDT working

Desirable criteria

  • Experience in a ward setting
  • Experience in older people's nursing
  • Experience of leading ward based projects
