Short Breaks Co-ordinator (Family Placement Team)

Family Support Team Short Breaks Co-ordinator at
Pembrokeshire County Council .
Key Duties Will Involve
- Organising short breaks and coordinating care packages
- Conducting risk assessments and detailed observational reporting
- Engaging with multi-agency teams to support carers and families
- Supervising short break carers and providing ongoing guidance
- Delivering information sessions and contributing to carer assessments
- Responding to emergency requests and supporting crisis intervention
In addition to relevant qualifications, the ideal candidate will have experience working with children and families affected by complex needs. They will be able to work independently and as part of a collaborative multi-agency team. Driving licence and access to a vehicle is essential.
