In this exciting role, youll take the lead in managing and motivating a skilled nursing team, ensuring the delivery of exceptional, patient-centred care. Youll champion health promotion, empower shared decision-making with patients and carers, and work closely within a dynamic multidisciplinary team to help individuals achieve their full health potential.
As a key clinical leader, youll oversee a diverse caseload, guiding your team to deliver safe, effective, and compassionate care. Youll play an essential role in supporting patients with both acute and long-term conditionsmonitoring their progress, evaluating treatment responses, preventing unnecessary hospital admissions, and promoting proactive, preventative care.
Youll also collaborate closely with our IV Therapy in the Community service, bringing specialist insight to enhance pathways and outcomes.

What youll be doing as part of our dynamic community nursing team:
Lead with excellence by upholding the highest professional standards in line with the NMC Code of Practice and all relevant guidelines
Deliver ho
listic, high-quality care by assessing, planning, implementing, monitoring, and evaluating personalised interventions for every patient
Be a confident point of contact , responding proactively to referrals, enquiries, and requests from the public and fellow professionals
Champion person-centred care by ensuring every patient on the caseload has a comprehensive, holistic care plan that is consistently followed by the team
Communicate with impact , building strong, compassionate relationships with patients, carers, and the wider multidisciplinary team to ensure an individualised approach at every stage
Drive continuous improvement by actively reviewing the effectiveness of nursing care and leading on any actions required to enhance outcomes
Support high-quality Integrated Community Nursing services for a defined population, contributing to the Trusts delivery plan and meeting key national targets
Shape the future of care by staying informed of Trust policies and actively contributing to the development and review of policies and procedures
Use your clinical expertise as a Nurse Prescriber , keeping up to date with regular training and prescribing safely and appropriately within the recommended formulary
Promote evidence-based practice by participating in research activities and ensuring research findings are translated into high-quality clinical care
Uphold an open and supportive culture by initiating and managing t
he Trusts complaints procedure when required, guiding staff, and ensuring thorough, fair investigations
