Are you a dedicated and skilled Social Care Practitioner looking to join a dynamic team within the Adult Social Care Service at West Berkshire Council? We are seeking an enthusiastic individual to work alongside our Team Leader OT, reporting directly to the Team Leader, to provide high-quality social care and support to our service users.
About The Role
As a Social Care Practitioner, you will be responsible for:
- Working collaboratively with the Team Leader OT to ensure effective delivery of social care services.
- Conducting assessments and reviews of individual needs, ensuring appropriate care and support plans are in place.
- Providing advice, guidance, and support to service users and their families.
- Collaborating with other professionals and agencies to ensure holistic care delivery.
- Monitoring and evaluating the progress of service users, making necessary adjustments to care plans.
About You
The ideal candidate will possess the following skills and attributes:
- A relevant social care qualification and registration with the appropriate professional body.
- Experience working in a social care setting, ideally within an adult care environment.
- Sound knowledge of relevant legislation, regulations, and best practices.
- Excellent assessment and care planning skills.
- Strong communication and interpersonal skills, with the ability to build positive relationships with service users and colleagues.
- Ability to work effectively both independently and as part of a team.
- Good organisational and time management skills, with the ability to prioritise tasks.
