Reablement Occupational Therapist (Band 6/7)

Location: Haringey, London

Contract: Full-Time

Pay Rate: Up to £38 per hour

Sanctuary Personnel is recruiting an experienced Reablement Occupational Therapist to join a community-based reablement service in Haringey. This is an excellent opportunity for an HCPC-registered OT who is passionate about supporting adults to regain independence following hospital discharge and thrive within a multidisciplinary team.

Role Overview

As a Reablement OT, you will work with clients in their own homes following discharge from hospital, delivering high-quality occupational therapy assessments and interventions. The role focuses on promoting independence, reducing reliance on long-term care, and supporting safe, timely transitions from hospital to the community.

Key Responsibilities

Undertake comprehensive occupational therapy assessments within a reablement and community setting
Complete regular ADL assessments and develop tailored reablement plans
Support service users to set meaningful goals and regain independence

Work independently with clients, families, and carers

Contribute effectively to MDT working, including liaison with health and social care professionals

Recommend appropriate care and support packages based on assessed needs

Maintain accurate, timely, and compliant case records and documentation

Travel across the borough of Haringey to complete community-based visits

Requirements

Qualified Occupational Therapist
HCPC registration (essential)
Experience in reablement, community OT, or hospital discharge settings
Strong knowledge of ADL assessments and reablement-focused interventions
Ability to work autonomously and manage a varied caseload
Excellent communication and report-writing skills
Full UK driving licence and access to a car (essential)
