Role Overview
As a Social Worker in the 18-25 Transitions Team, you will play a crucial role in supporting young adults as they transition into adulthood. Your responsibilities will include:
- Complex Case Work: Conducting Mental Capacity (MCA) Assessments, Court of Protection applications, and s42 safeguarding enquiries.
- Positive Risk Support: Encouraging individuals to take positive risks while aiming for the least restrictive interventions.
- Collaborative Working: Partnering with service providers and stakeholders to deliver comprehensive support.
- Strength-Based Approach: Undertaking complex assessments, setting personal goals and outcomes, and reviewing their effectiveness.
- Person-Cantered Decisions: Ensuring the individual's wishes are at the heart of every decision.
- Resource Management: Exploring community and commissioned services to meet eligible needs within budget.

Location
15 mins south of London Bridge, we are based in Catford SE6 with 2 mainline train stations 5-10 minutes’ walk from the office Laurence House and numerous bus routes that stop outside the offices. Whilst the service is primarily based in Laurence House, we operate a hybrid working approach as determined by service requirements.
Special conditions
- This post is subject to registration as a qualified Social Worker with Social Work England.
- This post is subject to an Enhanced Disclosure application to the Disclosure and Barring Service.
- Qualified Social Worker BA /DipSW/CQSW or equivalent and has completed ASYE programme.
- The appropriate standards are set out in the Job Description/Person Specification.

Salary
£44,430 - £46,437 (SCP 31 - 33) per annum
N.B: All appointments will be offered at the minimum spinal column point (SCP) for the grade of the role. The only exception to this will be when matching current salary (up to the maximum of the scale), which is subject to evidential proof.
