An exciting temporary opportunity has arisen to be part of Sheffield community based Integrated Care Team-Therapy, based in the North of the city at Firth park. This is a city-wide service, operating 8.30 am - 6 pm, 7 days a week (including Bank Holidays).

The post is a secondment or fixed term 22.5 hours per week for 9 months until the end of June, happy to negotiate days of work.

You will have experience in treating older adults supporting them to remain independent in their own homes and be passionate about patient centred care and delivering what matters to the person ,enabled by developing a close network of peers across sectors at neighbourhood level.

The service works to facilitate early discharge of patients from hospital and supports patients at home to prevent unnecessary hospital admission.

The successful candidate will provide community-based therapy for patients with a variety of conditions including neurological, orthopaedic, frailty, musculoskeletal and anxiety issues.

This post will be based within a well-established, multi-disciplinary team consisting of: Occupational Therapists, Physiotherapists, Specialist/Senior Therapy Assistants, Administrative staff and with access to mental health nurses .

The successful candidate will be supported to deliver the best care via a robust and caring leadership team with a compassionate and collaborative focus.

The successful candidate will carry out generic and Occupational Therapy assessments.

This will include planning and delivering treatment programmes for patients in their own homes including residential care settings.

The role will involve working closely with therapy assistants; contributing to their training and development and facilitating them to provide care and rehabilitation to a designated caseload.

The role also involves transporting, providing, ordering and fitting of equipment and onward referral to other health and social care services.

The role will support student placements and will enable Occupational therapy students to understand the value of community integrated care.

The role will support the service as needed with specific audits and research enabling the service to improve its service offer to the people of Sheffield.

The role is supported by a band 7 duty lead and a band 7 clinical specialist Occupational therapist ensuring support with day to day issues and complex clinical situations.

This vacancy is advertised on a fixed term contract basis for a period of 9 months until June 2026 due to the post being recruited as part of a limited term project within the Trust. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the requirement for the post to be maintained as a part of the project’s implementation and the timescales associated with this.
