Specialist OT LiveWell AgeWell

The LiveWell team are looking to recruit a team leader to develop our exsiting project to support frail residents of Cardiff and the Vale to self manage and remain in the community healthier for longer. The main duties and responsibilities will be around managing the LiveWell AgeWell programme and supporting the MDT to deliver high value high impact interventions. The post holder will work collaboratively with CRT's and community partners across the health boards geographical area. They will be managing a team of support staff who they will support to deliver this preventative programme

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
