An exciting opportunity has arisen for an enthusiastic and motivated pharmacist to join the well respected and progressive Anticoagulation Clinical Pharmacy Team at King’s College Hospital.

We are an exemplar centre for venous thromboembolism and have a strong research element to our pharmacy team. This post and provides an excellent opportunity for a pharmacist to develop their clinical pharmacy and managerial skills in the highly integrated, multidisciplinary thrombosis team at King’s College Hospital NHS Foundation Trust.
Reporting to the Principal Pharmacist Anticoagulation, your responsibilities will include the following key roles:
- Deputise for the Principal Pharmacist Anticoagulation
- Lead and manage clinical pharmacy services in Anticoagulation in conjunction with the Principal Pharmacist
- Participate in multidisciplinary ward rounds delivering and promoting high quality, cost-effective, patient-focused pharmaceutical care
- Consult with primary care colleagues to ensure the safe initiation and management of anticoagulation
- Act as a specialist role model and an expert in the prevention and treatment of thrombosis and the management of anticoagulated patients
- Supervise and train rotational pharmacy staff
- Ensure a high level of clinical pharmacy input to out-patients in the anticoagulation clinics and in-patients on the multidisciplinary ward round
- Promote safe drug use for anticoagulated patients across all specialities
- Develop, implement and audit local guidelines
- Education and training of healthcare professionals within and outside the Trust relating to the safe use of anticoagulants
- Patient/carer education as part of discharge planning and close liaison across the interface
- Participate in practice and clinical research and audit
- Work with the Principal Pharmacist Anticoagulation to extend the boundaries of professional practice within the anticoagulation team
