Description
The Active Programmes team-provides rehabilitation to clients with long term conditions in the community.

Work as a team to deliver high quality, patient focused physiotherapy treatment programmes care for patients with respiratory and cardiac long-term condition across Sheffield community venues and within their own home.

Manage a caseload of patients with all types of Cardiac and Pulmonary related conditions using

specialist skills carrying out holistic assessments and implement a patient centred plan and evaluate the clinical interventions which address the patient goals with the agreed treatment protocols and best practice documents.

To promote healthy living in patients with long term conditions.

To supervise and train inexperienced physiotherapists, assistants and students working within the service.

To be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure a high standard of clinical care for the patients under your management.

To frequently undertake the comprehensive assessment of patients, with a complex presentation, requiring prolonged concentration to investigate and analyse the assessment to formulate individualised management and treatment plans, using clinical reasoning and a wide range of treatment skills and options to formulate a specialised program of care.

To use continuous assessment to evaluate treatment outcomes and adjust treatment plans, accordingly, appropriately referring to other agencies to meet patients’ needs and promote integration of services.

To seek clinical and professional advice and support from senior/specialist physiotherapists when required.

To undertake training to develop and demonstrate competency to practice as a specialist in cardiac and pulmonary rehabilitation.

To accept clinical responsibility for patients and to organise this effectively and efficiently with regard to clinical priorities and use of time management.

This vacancy is advertised on a fixed term contract basis for the maximum period until 30/09/2026. Where there is potential, the secondment/fixed term contract may finish early, or in the first instance, the secondment/fixed term contract may be extended due to the internal/external secondment of the substantive post holder. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and secondments in relation to the substantive post holder.
