Description
The successful candidate will work as part of a dynamic and flexible multidisciplinary team, delivering high-quality, responsive care to patients across the Virtual Ward. The role also includes in-reach responsibilities within the acute hospital and Accident & Emergency departments to identify patients suitable for step-up or step-down care within the Virtual Ward.

There is an expectation of flexibility to work across the Virtual Ward, Urgent Community Response, and acute hospital settings, supporting seamless transitions of care and ensuring patients receive the right care, in the right place, at the right time.

Key Requirements

Registered Nurse with current NMC registration

Level 7 Advanced Health Assessment qualification

V300 Non-Medical Prescribing qualification, with a minimum of 12 months prescribing experience

Proven clinical expertise in managing patients with complex needs/acute experience/managing the acutely unwell patient.

Excellent communication, clinical decision-making, and leadership skills

Ability to work autonomously and collaboratively across services

Additional Information

As this is a community-based role, a full UK driving licence and access to a vehicle are essential.

As a Band 7 Nurse Practitioner within the Virtual Ward, you will deliver autonomous, advanced-level clinical practice that is firmly grounded in direct, compassionate care for patients, families, caseloads, and populations. Your role will encompass elements of clinical leadership, education, research, and service development, while maintaining a strong focus on patient-centered care.
