Assesses and treats own specialist caseload of patients/clients and maintains associated records. To develop and maintain links with local specialist patient support groups. To liaise with dietitians within the team and ensure overall prioritisation of workload within the team supporting other members as appropriate. Works in a variety of both inpatient and outpatient settings, which will include satellite units responding to referrals in a timely manner as per clinical need.

Provides patient centred nutritional care, tailored to patient needs, ability and understanding. Provides continuity of care across stages of chronic kidney disease (CKD).
To work as part of the multi-disciplinary team providing therapy input to patients in renal and to liaise with other disciplines and agencies as required in the planning and implementation of patient care. To liaise with and work as an integral member of the multi-disciplinary team, building and maintaining effective working relationships within the Trust and the health economy. Liaise with dietitians from other Trusts and other Healthcare professionals concerning patient treatments and progress with dietary regimens, depending on the patients clinical condition and need.
