Job Summary
As part of our Service Redesign, new posts have been developed to support our high risk patients who require a fast response in order to prevent hospital admissions where possible. This is an exciting opportunity for an innovative and experienced therapist to join the countywide Worcestershire Adult Speech and Language Therapy (SLT) Service, which covers Community Hospitals, Out-patient Clinics and Specialist Teams, with the aim of providing the optimum evidence-based service for the people of Worcestershire.
Full time fixed term 12 month Countywide post (part time or job share considered; minimum 0.4 WTE) will focus on Fast Response under the guidance of a Highly Specialist Clinical Lead, as well as working alongside experienced community SLT colleagues. Clinically, you will provide specialist assessment, diagnosis and case management for patients with a range of communication disorders and dysphagia related to a variety of diagnoses and may maintain a small caseload.
You will be integral to the continued development of the Service into a high performing, well respected provider of timely, safe and effective Speech and Language Therapy for our patients, therefore we require an ambitious and creative clinician who is self-driven, and who also works well within a team.

Main duties of the job
A typical day will involve managing a caseload of fast response referrals in liaison with the Clinical Lead, prioritising visits across the county, raising any potential concerns regarding capacity and demand. You may also maintain a small general caseload based in the north of the county, working at a Specialist level, as well as having the opportunity to work clinically in inpatient settings.
You will work independently in the assessment and management of patients with communication and swallowing disorders, completing clinical documentation on our electronic records system.
You will formulate individual therapy programmes targeting realistic patient-led goals, as well as offering support and education to carers/families.
You will liaise with other members of the team and MDT colleagues; support student placements; provide in service training for SLT colleagues and other professions in areas related to SLT, and participate in audits as required by the service and the Trust.

You will receive training and supervision, and will engage in self-directed learning.
