Job Overview
An exciting opportunity has arisen for an enthusiastic, self-motivated, and forward-thinking Staff Nurse to join our expanding team in the Discharge Lounge at Wexham Park Hospital.
This is a key role supporting patient flow and capacity across the hospital. As part of a well-established and growing team, you will be responsible for providing high-quality nursing care to patients who are medically fit and awaiting discharge. You will work both independently and collaboratively with the multidisciplinary team to ensure patients are safely and efficiently transferred to the Discharge Lounge, helping to streamline the discharge process.
The successful candidate will demonstrate excellent communication, organisational, and clinical skills, along with a flexible and proactive approach to patient care. You will be working in a dynamic environment that requires the ability to prioritise and respond to the changing needs of the service.

Main duties of the job
- The post holder will play a key role in promoting and facilitating the safe and effective use of the Discharge Lounge.
- They will be responsible for identifying patients who are clinically appropriate for transfer, liaising closely with ward teams and multidisciplinary staff to ensure timely and appropriate referrals.
- The role involves actively encouraging ward staff to utilise the Lounge as part of the hospital’s discharge planning process, helping to improve patient flow and bed availability.
- The post holder will ensure that all patients transferred to the Lounge have a confirmed discharge plan in place, and that their safety, dignity, and ongoing care needs are consistently maintained.
- They will also provide direct nursing care and supervision while patients await discharge, acting as a point of contact for relatives, transport teams, and community services.
- This position requires excellent communication skills, sound clinical judgement, and a flexible, proactive approach to supporting patient-centred care.
