An exciting opportunity has become available for motivated individuals to join our Frailty unit on ward 10 PCH.
The successful candidates will be working within a 24 bedded unit where you will be caring for patients with various medical conditions.
The aim of the unit is to appropriately assess patients, to facilitate discharge to a safe suitable environment .
The successful candidates will be working within a large MDT including PT/OT and SALT.
This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. Caring for a group of patients with various medical conditions.
Identify any health needs using a multidisciplinary approach and put plans in place to get patients home as soon as medically fit, to prevent deconditioning and getting infections.
Other Duties Include
Administration of medications.
Performing observations including neuro obs.
Pressure area care.
Assessing pain.
Discharge planning . D2RA Pathway.
Risk assessments/care planning.
Documentation.
