As part of the London sickle cell transformation programme, we are seeking to appoint a highly motivated innovative and enthusiastic transition nurse. The post is based at the Royal London hospital, but will require working jointly with hospital and community teams across the North East London (NEL) Network to enable a smooth transition to adult services.

[email protected]The post holder will be an advocate for children and young people transitioning to adult care in Barts Health and hospitals that are within the East London and Essex networks. This will include
- Working closely with the multidisciplinary team to provide support, advice and education for teenagers and young adults in inpatient and outpatient setting up to the age of 25years
- Working across paediatric and adult services within the trust.
- Acting as a resource for teaching and educating patients and staff
- Developing CYP strategic priorities
