nowledge to undertake an advanced clinical pharmacist role in the provision & development of pharmaceutical services, to patients under the care of medical teams within the urgent care division.
To review discharge processes to maintain and improve patient flow.
To provide expert advice and guidance on the safe and effective use of medicines for patients, carers, and other healthcare professionals within the urgent care division.
To provide education on aspects of pharmaceutical care for pharmacy staff and other staff disciplines (e.g. nurses, junior doctors, PAM’s).
To monitor compliance with NICE guidance and TAs, and other locally agreed guidelines and pathways relating to the urgent care division.
To assist in the development of policies, guidelines and pathways relating to drug therapies used within the urgent care division.
Where appropriate, to prescribe medication as a non-medical prescriber, within areas of expertise, in accordance with a Trust approved personal formulary.
Where appropriate, to work as an independent practitioner, examining patients & developing a management plan, in accordance with Trust approval.
The Trust’s services are provided from three locations:
- The Countess of Chester Hospital: providing 473 general and acute beds.
