Advanced Clinical Practitioner - Frailty

Surrey and Sussex Healthcare NHS Trust is expanding its Frailty and Same Day Emergency Care (SDEC) service and is looking for ambitious clinicians to join us as Advanced Clinical Practitioners (ACP) or Trainee ACPs.

You’ll work at the clinical front door for older people living with frailty, using advanced practice skills to make high‑impact decisions, improve patient flow and deliver safe, person‑centred care—often at pivotal moments in a patient’s journey.

The role

You’ll carry out advanced assessment, diagnosis and management of patients with frailty, applying expert clinical reasoning and collaborative decision‑making. The role offers variety, challenge and the opportunity to shape pathways as the service continues to grow.

• ACP (Band 8a): Practise autonomously at advanced level, contributing to clinical leadership, service development and high‑quality frailty care.

Trainee ACP (Band 7): A structured development role with strong supervision and support, including completion of a Master’s‑level Advanced Clinical Practice programme and clinical portfolio, progressing to Band 8a on achievement of ACP competencies.
