As a Advanced clinical practitioner within the practice, you will play a senior role in supporting the wider team. Advanced practitioners form a key cornerstone of clinical stability within the service. You will help provide experience, leadership, and support for colleagues while helping the practice maintain safe and effective patient care.

JOB SUMMARY
The post holder will be an experienced Advanced Clinical Practitioner acting within their professional boundary to provide autonomous care for presenting patients. This role is expected to develop and evolve over time as the successful post holder gains experience.
The primary care practitioner will act as an independent clinician, managing a broad range of patient presentations in both the practice and community settings. They will utilise their clinical expertise, including independent prescribing, to assess, diagnose, treat and manage patients effectively.
The post holder will attend home visits, including to residential and nursing homes, working collaboratively with staff, families and other stakeholders to produce and share care plans, including with Out of Hours (OOH) services.
They will also assess and treat acute patients presenting in the surgery, both face-to-face and via telephone consultations. In addition to providing high-quality, evidence-based care, they will contribute to the development of clinical practice through mentoring and supporting colleagues and students.
