We are on the lookout for a skilled and motivated Approved Mental Health Professional to join our dynamic Frontline and Community Teams right in the heart of Surrey, UK. This exciting full-time position offers a salary of £49,399 to £52,588 annually. What you will do:

- Lead and complete holistic assessments of the social care needs of individuals in line with the Integrated Care and Health Pathway.

- Work in partnership with individuals, carers, and professional staff from various agencies to establish services that meet identified needs and promote independence.

- Manage a caseload that requires additional support or guidance, focusing on identifying needs, minimizing risk, and promoting best interest decisions.

- Promote the recovery model in your practice and support health colleagues in adopting this approach.

- Conduct assessments and reviews for individuals, carers, and families, ensuring their needs are addressed and they are safeguarded.


Work Context:

Based in a community and hospital setting, you’ll be traveling across the picturesque Surrey landscape to support clients in their homes or care settings. You'll collaborate closely with multi-disciplinary teams and will be under the supervision of a Senior Approved Mental Health Professional or Service Manager. As an integral part of the team, you’ll work with diverse groups, including older people, those with learning disabilities, and individuals with mental health needs, to bolster independence and empower individuals.


Requirements:

We require a professional social work qualification as an Approved Mental Health Professional, with HCPC registration and extensive experience in mental health or social care settings. A solid understanding of the Mental Capacity Act 2005 and other relevant legislation is essential. You should possess excellent communication and IT skills, a knack for professional judgement, and the ability to build effective partnerships in multi-agency environments. Experience in providing reflective supervision will be a distinct advantage.
