We have meaningful conversations that start with people's own strengths, and making connections with families, friends, and local communities to achieve the outcomes that matter and improve wellbeing. When care and support is required, we work collaboratively with people to find solutions to live their lives as independently as possible. Based in either the First Contact and Early Support team or Localities team, you'll visit people in their homes, listen to what matters to them, and work with them to find practical solutions that make daily life easier. With experience of working with adults who draw on support, you'll be able to work in a busy fast paced environment and thrive under pressure. You'll have strong communication skills, with the ability to work both independently and as part of a multidisciplinary team. Demonstrating empathy, you'll be committed to personalisation, prevention, and empowering residents. Appointment will be subject to an Enhanced DBS check.
