Description
An exciting opportunity has arisen for a full time Band 6 Community Mental Health Nurse within the Complex Intervention and Treatment Team (CITT) in North Somerset. We are an integrated health and social care team that provides support for later life service users across the locality with both functional illness and dementia.

You should be an organised, highly motivated and enthusiastic individual who has excellent communication skills, a flexible approach to work and be committed to working with later life service users. Previous experience of working with this client group would be an advantage. We are a very supportive team offering a range of opportunities for supervision.

We would welcome anyone who would like to come along for an informal visit.

You will be a key worker and will co produce Safety Assessments and Personal Wellbeing Plans with your service users and their families/carers.

This will include signposting to and navigating community resources

You will be completing Care Act assessments, if indicated, using the local authority IT system LAS.

We operate a duty system, staffed by 1 qualified person each day.

Attending team meetings and contributing to case discussions.
