Do you have strong coordination and communication skills? If so, we want you to join our team as a DSR Coordinator.
This is an exciting opportunity to play a key role in developing and delivering the Dynamic Support Register (DSR) across Birmingham and Solihull. You’ll help services/providers identify individuals at risk of hospital admission and ensure they receive the right care in the community. Working closely with multi-agency partners, you’ll keep the DSR live, accurate, and impactful.

What We’re Looking For
- Degree-level qualification in health/care or equivalent experience
- Experience in multi-agency working and healthcare for people with autism/learning disabilities
- Strong organisational, data analysis, and ICT skills
- Ability to work autonomously and manage complex situations
- Manage referrals and monitor BSOL Citizens on the DSR
- Coordinate multi-agency meetings and responses
- Provide education and training on DSR processes
- Support national reporting requirements and local transformation programmes
- Act as a specialist in DSR pathways, embedding best practice
