An exciting opportunity has arisen for experienced and motivated Band 7 Clinical Leads to join our Neighbourhood Mental Health Team within the West Birmingham Primary Care Networks (PCNs).
We are seeking enthusiastic, dynamic clinicians who thrive in a multi‑disciplinary environment and are committed to delivering high‑quality, person‑centred care. You will work closely with a range of professionals, including GP colleagues within PCNs, to support improved outcomes for individuals across the local communities we serve.
Our community mental health services are well established, and as part of the BSOL Community Transformation Programme, we are expanding our neighbourhood teams to strengthen our presence within primary care. This is a unique opportunity to contribute to national developments in community mental health and to play a key role in shaping innovative, integrated models of care.
The continued development of our Neighbourhood Mental Health Team is central to delivering the ambitions of the NHS Long Term Plan, and we are committed to evolving our services to meet the needs of our population.
You will be responsible for providing mental health triage, assessment and outcomes-based Intervention for service users within the Neighbourhood mental health team.
You will be responsible for identifying need and best course of advice, support or intervention.
You will be responsible for promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
Crucial to this role will be your ability to ensure robust communication across FTB/BSMHFT and primary care in accordance with each individual service users’ needs by provide support and advice to all colleagues with the PCN as required including participation within the local neighbourhood MDT's and those working in the voluntary sector.
