Main duties of the job
To provide rehabilitation support services.
To undertake specific skilled support work to aid the rehabilitation of patients in the Community. Treatment may occur in the department, patients home or other appropriate setting. Treatment may be provided for Physiotherapy, Occupational Therapy or Speech and Language Therapy.
To carry own patient caseload, assessing and treating routine patients within guidelines agreed by the treating therapist. Monitor progress and make minor alterations to patient treatment programmes as required.
Work unsupervised reporting back patient progress and informing the treating therapist of any problems. Receiving guidance and advice on an almost daily basis.
Detailed job description and main responsibilities 1. To gather information prior to seeing client and provide feedback to the therapist:
- To obtain biographical information and knowledge from relevant sources of information, e.g. medical notes, computer records.
- To take a simple case history.
- To explore other sources of external information relating to patients.
- To contribute to the assessment process and provide feedback to the therapist:
- To carry out mobility and transfer assessments for specific pieces of equipment as requested by therapy or nursing staff and report back to qualified member of the team regarding the need for further input.
- To implement therapy programmes/approaches, both on an individual basis and for groups and provide feedback to therapist.
- To carry out programme devised by the therapist using a range of delegated exercises and manual physical therapy techniques.
- To work jointly with clinicians in cases which are complex and may require physical moving and handling as part of their rehabilitation.
- To plan patient treatment programmes for the approval of the therapist and implement.
- To monitor progress and to implement changes as appropriate keeping the therapist informed.
- To undertake support work alongside the therapist for highly dependent patients with complex needs.
- To be responsible for assessing and managing clinical risk in relation to your own caseload, seeking guidance and help from therapist as appropriate.
- Will not attempt any procedure without appropriate training and deemed competent to carry out specific instructions.
- To contribute to the evaluation of therapeutic interventions, including discharge planning :
- To contribute objectively and comment on observed therapy intervention, using an outcome measurement for a specific goal.
- To be able to adapt the treatment programme according to the outcome measurements.
- To be able to prioritise workload, following managerial supervision.
- To prioritise and plan day-to-day workload.
- To identify factors involved in prioritisation and to contribute to discussion about long term prioritisation of work.
Person Specification Skills Essential Criteria
- Experience of working with the public in a variety of settings and able to demonstrate good customer care. Experience of working in a health care setting, with skills and experience in rehabilitation of patients, general mobility skills and communication Good Verbal and written communication skills
Desirable criteria
- Good organisational skills Ability to work on own initiative & as part of a team
education Essential criteria
- 5 GCSE's at Grade C or equivalent including English language and Maths
