Are you a qualified Psychiatric Nurse passionate about mental health?

Do you wish to work for a team that is going through an exciting time in terms of changes to the way we work in line with the Community Mental Health Framework?

Oak team, within the City and NE Oxon AMHT have an opportunity for you to become part of the well-established service covering a diverse population within Oxford.

The City AMHT consists of 3 teams covering aligned GP practices across the city. It consists of a multi-disciplinary team of a range of different clinical staff members. We also work closely with embedded third sector services in each locality.

You will be supported by an experienced leadership team from across the professions who facilitate forums for Nursing and other allied health professionals and hold your development as central. We offer regular supervision and you will be encouraged and supported to undertake any training relevant to your professional development.

The role is 37.5 hours per week, Monday to Friday 09:00 - 17:00 - although there may be opportunities to vary this a bit.

The ability to travel independently between sites within the Trust is essential for this role

The role will involve being the allocated key worker (formerly known as Care Coordinator) for a caseload of patients with severe and enduring mental illness. In these instances, you will be playing an integral role as part of a multi-disciplinary team in that person's recovery, which would include the assessment, collaborative care planning and review of the person's needs.

You will also be expected to undertake bio-psycho-social assessments for patients referred to the team, and this may include offering brief evidence based interventions for up to eight weeks, ensuring to work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.

You will also be expected to complete duty, which includes triaging new referrals (we receive referrals from the Keystone Hubs, GPs, EDPS etc) and identifying if the patient requires an assessment within the AMHT or signposting to other services.

You will be expected to be able to complete risks assessments and appropriately escalate any concerns about risk or safeguarding.
