We are looking for a Senior Nurse, experienced working with patients in the community with complex long-term conditions.
Covering as a secondment or fixed term contract you will join with our Liskeard and Looe Community Matron Team who, as part of the wider Community Nursing Team,work closely across Adult Community Services, Adult Social Care and Primary Care, using enhanced clinical skills to enable patients to remain independent at home for as long as possible and with the aim of reducing the risk of avoidable hospital admissions.
We are looking for someone who has completed the HEAB 358 Advancing Practice in Long Term Conditions Management module or the equivalent. The post holder should be an independent prescriber or working towards this qualification
The post holder will need a high level of communication, problem-solving and decision-making skills.
This is an exciting opportunity as you will be expected to work in partnership with others to develop, take forward and evaluate direction, policies and strategies as we seek to expand our proactive frailty approach within the North Kerrier Neighbourhood Team.
