Description
Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Now is the time to be part of a healthcare transformation. The new NHS 10-Year Plan puts community care at the centre of service delivery—shifting resources closer to where people live and promoting prevention over hospital admission.

As a member of a community-based NHS trust, you’ll be at the forefront of this change. Work in integrated neighbourhood teams, delivering joined-up care alongside GPs, mental health professionals, social workers, and voluntary services. Benefit from new roles, clear career pathways, apprenticeships, and personalised development support.

The plan also brings new workforce standards: improved flexible working, mental health support, safe workplaces, and strong protections against discrimination. With greater investment in underserved areas and a focus on reducing health inequalities, your work will have local impact and national importance.

Community trusts are leading the way in digital innovation, using tools like virtual wards to reduce admin and improve patient care.

If you’re passionate about making a difference, this is your chance to help deliver a modern, inclusive, and community-focused NHS.

The Dover Community Nursing Team provides hig
h‑quality nursing care to adults aged 18 and over in their own homes, supporting people with complex and long‑term conditions to remain safe, well and independent in their chosen environment. Our service delivers a wide range of interventions including wound management, palliative and end‑of‑life care, symptom control, catheter care, venepuncture and the administration of injectable medications. We are committed to delivering personalised, holistic care that prevents unnecessary hospital admissions and supports patients to live well at home.

As a Band 6 Community Nurse Team Leader, you will play a key role in leading and supporting the team while managing your own caseload of patients with varied and often complex needs. Your daily work will include providing expert clinical care, overseeing wound dressings, catheter management, medication administration, VAC therapy and end‑of‑life support. You will guide and mentor students and junior staff, helping to build skills and confidence across the team. You will also contribute to service delivery by supporting colleagues, coordinating care and ensuring high clinical standards are maintained.

This role is ideal for an experienced community nurse who enjoys both hands‑on clinical work and team leadership. You will bring strong assessment skills, a proactive approach and a commitment to delivering compassionate, patient‑centred care.
