This is an excellent opportunity for a Band 5 Clinician to join a busy LMHT, supporting patients to recover and remain well in their own homes. You will be based in a diverse team and manage a caseload of patients from the City South area. Your role will involve being a Key Worker, planning , facilitating and reviewing patient centred care, involving their family, carers and external agencies. The ideal candidate will be a team player, able to manage a caseload and work autonomously, contribute their ideas to the MDT and build therapeutic rapport with patients. You will support on duty, as well as supporting junior staff and students.
The Local Mental Health Teams provide mental health services for people aged 18 to 65 years across Nottingham City, Nottinghamshire County and Bassetlaw. There are eleven LMHT, each of which includes the following specialist staff: Mental Health Nurses, Occupational Therapists, Psychologists, Psychiatrists, Social Workers, Community Support Workers, Peer Support Workers and Employment Specialists.
Our LMHT bring together a variety of services including Assertive Outreach, Community Rehabilitation, Community Assessment Treatment Services, City Recovery Service, Social Inclusion and Wellbeing, Community Occupational Therapy Service and Medical Services.
We help patients and their families to cope with periods of severe mental illness and assess, plan and implement a personal path of recovery for each patient. We work with people who have a wide range of mental health difficulties. Some may require short term intervention, and sometimes longer-term care plans will be put in place.
